Smart Implants in Dentistry: A Comprehensive Review of Emerging Technologies and Future Directions

Abstract
The field of dental implantology is undergoing a paradigm shift from passive mechanical replacements toward interactive, adaptive "smart" systems capable of actively engaging with the oral microenvironment. This comprehensive review examines the evolution, current state, and future prospects of smart dental implants. We propose a hierarchical classification framework categorizing implant "smartness" into four levels: biocompatible, bioactive, bioresponsive, and bioadaptive. The review critically evaluates emerging technologies including sensor-embedded implants for real-time health monitoring, bioelectric implants powered by masticatory forces, nanotechnology-enabled multifunctional surfaces, and AI-integrated robotic placement systems. Clinical applications, translational challenges including power supply constraints, data security concerns, regulatory hurdles, and economic barriers are systematically analyzed. While smart implants demonstrate significant potential for improving osseointegration, preventing peri-implantitis, and enabling personalized patient monitoring, most innovations remain in preclinical or early clinical stages. This review concludes with forward-looking insights into next-generation bio-intelligent prostheses and recommends priorities for future research, including long-term clinical validation, standardization of outcome measures, and development of interdisciplinary training frameworks.
